Description:

Brookline Council on Aging in conjunction with the Brookline Police Department has implemented the Brookline Home Safe Program, a community service for Alzheimer's patients (or elders with any type of memory loss) and their families and caregivers. An ID bracelet is purchased by the patient and is worn by the patient for identification and locating family members. Registration forms may be obtained from the Brookline Police Department, Community Service Division.
