?Coordinate with area agencies and hospitals to establish the most effected care at home, at the lowest cost to the elder.
?Assist legal guardians and families living at a distance with appropriate guidance and advice in order to ultimately promote a safe living environment (Nursing home, assisted living facility, or own home.)
?Offer conflict resolution by facilitating family meetings when families are not able to agree on the best choice for their loved one.
?Provide transportation to doctor appointments and consult with the physician.
Sheryl Fappiano, Owner/Manager, is a professional Geriatric Care Manager (GCM) working in all of Western MA to improve the safety of elders in their living environment. She is a licensed social worker, specializing in Gerontology. She has worked for over twenty five years in a variety of settings, all directly relating to Geriatrics. Along with working as a professional care manager for more than four years, Sheryl has worked for eleven years as a medical assistant in a physician’s office. Sheryl’s experience in the medical field has increased her awareness of illnesses and conditions often associated with the elderly. After starting her own business in 2003, Sheryl has naturally gravitated to helping families with conflict resolution and assisting many elders with End of Life issues. It is her honor to be such an important part of what it takes to help elders and their families’ transition through some very difficult times.
