Intake Procedure: A physician, case manager, or other clinician from the acute care hospital requests that the pt. be evaluated by a Fairlawn. Eligibility: Patients are usually referred following a hospital stay, may be referred from home. Hours of Operation: 24 hours a day/7 days a week.

Outpatient Specialty Programs & Services
The Fairlawn Outpatient Center, located on the hospital's easily accessible ground-floor level, provides a full complement of rehabilitation services as well as specialty programs. Those specialty programs include:
Functional Capacity Evaluations,
Hand Therapy Program,
Osteoporosis Program ,
Swallowing Disorders Program,
Total Joint Center.
Our Outpatient Center also provides the following specialty clinics and procedures:
Second Chance Stroke Screenings,
Botulinum Toxin (Botox) Injections,
Electro-Diagnostic Evaluations (EMG),
Videofluoroscopic Swallowing Studies.
Stroke Support Group - 4th Wed of each month 2-3:30
Brain Injury/Stroke Caregiver Support Group - last Wed. of the month 5-7 pm
