Call Home Care intake department and make referral.
Eligibility:
Age 60 & over, income criteria & needs assessment required for home care services. Adult Family Care accepts clients 16 or older for placement in homes. PCA program referrals for people over 50 who have a permanent or chronic disability & MA Health

Notes 1:
HVES is a private, not for profit organization, that is funded by the Executive Office of Elder Affairs. HVES is dedicated to fostering independence, long term partnerships and to providing services and resource options that will empower elders (60+) and their families. Staff can explain resources and provide referrals to many community, state and federal programs and services such as Home Delivered Meals, Homemaking, personal care, laundry and day Programs. In addition, Ombuds staff and volunteers can assist nursing home residents and their families to understand their rights and can assist them to advocate for their concerns. The Adult Foster Care Program (AFC) offers a housing and can match host families to adults who need a supportive family environment. Individuals needing to report situations of elders who are living in the community and are at risk or who may be experiencing abuse or neglect situations should call the ' Intake Department' for assistance with their concerns.
Notes 2:
Care Advisors can provide a free, one hour Elder Care Advice visit to answer questions about local and statewide resources that are available to elders. Care Advisors are also responsible for coordinating and arranging home care services for people age 60 and older . Highland Valley also provides: Information and Referral; Nursing Home pre-admission screening; Community Dining Centers; Money Management and Personal Care Assistance (PCA). Caring Community Map: HVES sponsors an informational web site for elders, their families and their communities. The website provides a format for elders to confidentially list a need for service or a need for a resource and in turn, community members are able to list their offers of service and resources that they wish to share with elders.
